counselor for Mental health apartment program

Job Summary Responsible for providing counseling and training to clients in a more independent and responsible living environment. This includes individual and group counseling, maintaining clinical records, networking with other community providers, assisting clients with funding and financial matters, assisting in the daily operation of the unit (e.g. light housekeeping, food and supply purchasing, etc.). Responsible to Program Manager.

Minimum Qualifications High school diploma or equivalent required. Prefer individual with work experience in adult residential rehabilitation setting serving clients with mental illness or dual disorders of mental illness and chemical dependency and with demonstrated ability to function independently. NYS motor vehicle license, safe driving record and availability of vehicle for work are required.

Wage and Benefits wage midpoint is $13.62/hour. Actual hourly salary depends on education and experience. chemical dependency Financial Case Manager

Job Summary Responsible for assisting and teaching clients how to identify their needs, access and maintain linkages to services. This includes services such as financial benefits programs i.e. DSS/SSI, Medicaid, housing, treatment and other services that support health and recovery.

Minimum Qualifications High School Diploma or equivalent and 4 years paid human service experience, OR Bachelors degree in human service related field and 2 years paid human service experience required. Prefer individual with work experience in an outpatient, case management, or residential setting, serving clients with chemical dependency or dual disorders of chemical dependency and mental illness, with demonstrated ability to function independently. NYS motor vehicle license, safe driving record and availability of vehicle for work are required.

Wage and Benefits wage range midpoint is $15.19/hour depending on education and experience. Job Summary Responsible for providing counseling and training to chemically dependent adult clients in a community residence. Duties include support to clients, monitoring the self-administration of medication as needed, record keeping, responding to emergencies, communicating with agency On-Call Consultants and performing other routine tasks as assigned. Job Summary Responsible for communicating and consulting with program staff regarding clients’ conditions and functioning. Responds to crises and emergencies. Provides counseling and support, including supervision of medication to clients, when indicated. Maintains security and safety of clients and facility. Oversees management of medication documentation, provides unit support in preparing rosters, schedules, and other community policies and sign-up sheets. Performs routine facility maintenance tasks and other related tasks as assigned by supervisor.

Minimum Qualifications High school diploma, clean NY State valid drivers license and safe driving record required. Some relevant paid human service experience preferred.
